Financial Policy and Agreement
The following financial guidelines have been established in order to clarify any questions:
The rate per 1-hour-session is $180.00, unless other arrangements have been made. Full payment is due at the time service is rendered. Payments may be made by cash or check. We also accept MasterCard, Visa, Discover, American Express, debit and/or credit cards. All returned checks will be subject to a $35.00 returned check fee.
We understand that questions may arise, that will require direct communication, outside of the scheduled appointment time. If this occurs, a phone call will be considered a consultation to make sure your questions/needs are addressed. There will be a charge of $45 for every 15-minute increment.
If you have health insurance and wish to file a claim for repayment, we will be glad to provide you with a receipt to submit with that claim. However, your insurance is a contract between you, your employer (when applicable) and the insurance company. We are not a party to that contract. If you have not already done so, it is best to call right away to inquire as to what your behavioral health benefits are. All benefits will be out-of-network. If you have any questions, I will more than happy to help.
When you schedule an appointment, I hold that time for you, which prohibits others from being able to schedule. Therefore, our policy is to charge for missed appointments or cancellations without a 24-hour business day (Monday-Friday) advanced notice. (Consideration will be given to unusual circumstances.) We ask to keep a credit card on file to charge the full session fee in case the above takes place. We will also use this card to charge, after our sessions together. Your signature gives us permission to charge the below card as explained above. If at any time you need to change this card, please let me know.
PLEASE NOTE: There is a 3.6% processing fee for all card charges.
